PARENTAL CONSENT, ASSUMPTION OF RISK, AND LIABILITY ACKNOWLEDGMENT

IMPORTANT: THIS DOCUMENT AFFECTS YOUR CHILD’S PARTICIPATION. PLEASE READ CAREFULLY.

Acknowledgment of Risk

I understand that my child will participate in activities provided by Loving Adventures LLC, including skateboarding, surfing, paddleboarding, and other activities provided by Loving Adventures LLC.

I acknowledge these activities involve risks, including but not limited to:

• Falls and collisions

• Contact with equipment, surfaces, or water

• Drowning (for water activities)

• Equipment malfunction

• Actions of other participants

I understand these risks may result in serious injury, permanent disability, or death.

Parental Consent

I give permission for my child to participate in these activities.

I confirm my child is physically able to participate and has no conditions that would make participation unsafe (or I have disclosed them).

Assumption of Risk

I understand and accept the risks associated with my child’s participation.

Liability Acknowledgment

I understand that under New York law, this document does not waive all legal rights, including potential claims related to negligence.

However, I acknowledge the risks and agree that participation is voluntary.

Concussion & Injury Policy

I understand that my child may suffer a concussion or other injury.

I agree that my child will:

• Be removed from activity if injury is suspected

• Require medical clearance before returning

Medical Authorization

In an emergency, I authorize Loving Adventures LLC to obtain medical care for my child if I cannot be reached.

I agree to be financially responsible for any care provided.

Rules & Behavior Agreement

I agree that my child will follow all safety instructions.

I understand that failure to follow rules may result in removal from activities.
